Lottsford Council HOA

I've recently received the "LLG Recreation Association, Pool Opening Celebration" announcement and accompanying information concerning the pool pass and regulations for use of the pool.  Unfortunately the package didn't include the referenced "Registration Form" which is crucial to pool admittance.  Please contact me to let me know how I can obtain this form, before May 10, 2013 so that me and my family can gain entry to the pool.
