Application Forms

FINANCIAL NEED SCHOLARSHIPS: Partial scholarships are available for Suzuki lessons and group classes at the University of Louisville School of Music. Eligibility is based on federal school lunch program income guidelines. Applications should be submitted at least two weeks before the start of each semester's lessons. Please call Judy Hake at 502-852-5850 for an application form.

SUZUKI INSTITUTE FINANCIAL NEED AND MERIT SCHOLARSHIPS: Partial scholarships for the summer Suzuki Institute, based on financial need or merit, are available for violin, viola, and cello. A merit scholarship requires an audio or video recording of the student's playing. Please email  suzukistrings@louisville.edu for an application form.
