Mallard Pointe

I sent an email @ 2 weeks ago to mallard point asssocoation requesting a modification form so that i can obtain a shed and fence but have not heard back from anyone.  Does anyone know where to obtain this form as it is not on the site? I have 2 weeks left to complete the shed foundation and need to move on this.  Can i just send in a word document requesting a shed and fence along with a picture attached of the shed being purchased along with the property dimesions outlining where the fence will be constructed?  Please advise.
