E.J. May Kindergarten Round-Up

Tuesday May 8, 2007 from 9:30-11:30 in the multi-purpose room. Come register your child for the upcoming school year.

Please bring the following with you:
Birth certificate(original or certified copy)
Your child must be 5 years old on or before September 30, 2007.

Parent photo ID

Current Immunization Record

The company "Helping Kids" will be there to administer immunizations until 11:00 for a fee of $14.00.

Two current proofs of address
(i.e. mortgage papers, rental agreement, utility bills, NO phone bills, cable bills, or driver's license)

The above proof of residency must be in the parent or guardians name.
