All major architectural changes must be approved by the SVMHOA Board/Architectural Committee before commencement to avoid fines.  Send the above form to the SVMHOA Board of Directors/Architectural Committee at the above postal mail address or by e-mail to the e-mail address.  The Committee has up to 15 days to approve or disapprove the plans.  All improvements must be in line with City Codes.

I have read and understand the conditions and regulations of the Architectural Committee set forth
in the CC&Rs and the Architectural Control Policy for Summit View Meadows HOA.  I further understand that up to 15 days may be required for approval once the plans are submitted.
