MomCare: A Circle of Care for Pregnant Women

What is MomCare?

MomCare is a statewide Medicaid funded insurance program for pregnant women in the state of Florida.

To apply, complete the "Simplified Eligibility Form", a one-page application and mail with proof of your pregnancy from a healthcare provider to the local DCF office at 1111 12th Street, Suite 306, Key West, FL 33040.

You should be notified within 5-business days of acceptance.

Once you are accepted into MomCare, you will be assigned to the Monroe MomCare Advisor. She will assist you in selecting an obstetrician or midwife, discuss your needs and provide you with information on accessing any other services you may qualify for, including WIC.

The Simplified Eligibility Form can be found online at the Health and Human Services link below (click on the Health Insurance for Pregnant Women file) or from the Department of Children and Families offices.
